Who Should Fill Out the Intake Forms?

  • New patients and first-time visitors to Moyer Total Wellness
  • Existing patient who have not been into our office for 12 months or longer
  • Existing patients who have not filled out our intake forms in 12 months or longer (please complete these forms once a year and inform us of any changes in between so we can always have the most up-to-date and accurate information).

What Do We Do With This Information?

Your Intake Forms will be securely stored in your Moyer Total Wellness account and will only be accessible to Moyer Total Wellness staff.

I am an Auto Accident / Personal Injury Patient, Do I Need to Fill Out These Forms?

Yes, you will need to fill these forms out, however, there will also be additional paperwork to fill out before your first appointment which is only available in our office.

Please plan to come to your first AUTO appointment 30 minutes early to fill out all of the necessary paperwork. Don’t forget to check out our AUTO Insurance Information page to learn more about what you will need to bring to your first appointment.

*Very Important: At the time of booking, please discuss the type of massage or other service you are expecting when you schedule your appointment. This will ensure we match you with the most appropriate staff member for your needs.

We require a Credit Card on file to schedule any appointment at MTW. If you need to reschedule, please notify the office at least 24 hours prior to your appointment start time to avoid the Late Cancellation Fee. When possible, our massage therapists prefer to receive cash tips or tips through Venmo.
